LS Forms Library

The following Lake Summerset forms are available for downloading. They are in Adobe pdf format which is readable by most computers. If your computer doesn't have adobe Reader, you can download the reader for free from:

http://www.adobe.com/products/acrobat/readstep2_allversions.html

Just pick the language you want and operating system (Windows/MAC).

Click on the form below and it should open on your computer. Then print the form on your printer for mailing, faxing, or bringing to the LS business office.

Completed forms and payment can be returned to the LSA Business Office. Help on filling out the forms can also be obtained from the LSA Business Office at 815-248-2194.

  • Party List - This form is to be used when someone is having a party, wedding, etc., with over 10 names. (ALL GUESTS in vehicle must be listed. Party list must be turned in at least 24 hours in advance to main gate.)
